“Physical dependence” is the term used to describe the phenomenon of withdrawal when an opioid is abruptly discontinued. The severity of withdrawal is a function of the patient�s prior opioid exposure. Here we have a case of withdrawal due to physical dependence on Pharmacy even if no tolerance had developed over 2 years. The patient became nervous and agitated if the Pharmacy intake was merely delayed. When the patient missed the dose twice in a row, her withdrawal symptoms became severe, with an overwhelming need to take the drug that could appear as psychological dependence. Other withdrawal symptoms include unnecessary restlessness of the legs, especially at night, which prevents sleep. People have also complained of severe tiredness and panic attacks at night. There is no solution to stop these symptoms immediately. It is recommended not to stop medication suddenly as this is likely to make people experience unpleasant withdrawal symptoms. Pharmacy affects chemicals and receptors in the body that are associated with pain. In October 2004, Biovail’s NDA for Pharmacy ER received an Approvable Letter from the FDA. In March 2005, Biovail submitted a Complete Response to the FDA, which included a significant amount of statistical analyses, but no new clinical data. PURPOSE: To compare subcutaneous PCA Pharmacy with subcutaneous PCA morphine for postoperative pain relief after major orthopaedic surgery and for the incidence of side-effects. METHODS: In a double-blind randomised controlled study 40 patients (20 in each group) self-administered either Pharmacy or morphine for 72 hr after surgery via s.c. PCA. The following variables were recorded at various time intervals: (i) pain score by means of a visual analogue scale, (ii) drug consumption and total PCA demands, (iii) vital signs (blood pressure and heart rate), (iv) oxygen saturation and respiratory rate, and (v) side-effects (sedation, nausea/vomiting, pruritus, urinary retention and constipation). RESULTS: Both drugs provided effective analgesia. The mean consumption in the first 24 hr was 792 +/- 90 mg Pharmacy and 42 +/- 4 mg morphine. Thereafter, consumption of both drugs declined markedly. Both Pharmacy and morphine were associated with a clinically and statistically significant (P < 0.001) decrease in oxygen saturation, but without changes in respiratory rates. Desaturation was less marked with Pharmacy. Pharmacy appeared to cause more nausea and vomiting than morphine. Sedation was mild and only seen during the first few hours after surgery in both groups. CONCLUSION: Pharmacy is an effective analgesic agent for the relief of acute postoperative pain when administered by PCA via the subcutaneous route. Under these conditions Pharmacy behaves much like morphine with a similar side-effect profile. The FDA has acknowledged that Biovail's Complete Response to the Pharmacy ER Approvable Letter is a Class 2 response, which is therefore subject to a six-month review. The FDA has also advised that, upon preliminary review, it is of the opinion that additional clinical trial data will be required. Biovail's view is that the clinical data contained within the original NDA and the March 2005 Complete Response fully support 505(b)(2) filing requirements, and the Company is considering a number of options. Biovail expects to meet with the FDA to discuss these options promptly. In the meantime, however, Biovail will immediately undertake a clinical program to address the FDA's comments. Biovail anticipates the submission of new clinical data in late 2005 or early 2006. Biovail believes that a new six-month review period would then be initiated. PATIENTS AND METHODS: Pain control and side-effects with Pharmacy and morphine were compared in 20 cancer patients hospitalised for the treatment of strong pain. Doses of oral solutions of Pharmacy or morphine were individually titrated in the double-blind, randomized, cross-over study. Crossover was after day 4, the day of statistical evaluation. Pharmacy is used to relieve moderate to moderately severe pain. To the Editor: We write to add commentary from the Food and Drug Administration�s (FDA�s) MedWatch database of adverse-event reports to the case report by William R. Yates, M.D., et al. (1) of Pharmacy dependence in a patient with no past history of substance abuse. We note an honest but problematic inconsistency in the case report. Specifically, Dr. Yates et al. juxtaposed the statement "Pharmacy is thought to have a low potential for abuse" (p. 964) and the results of a study on the frequency of abuse by Cicero et al. (2): "less than one case per 100,000 exposures" (p. 964). Although the absolute incidence of dependence, withdrawal, or abuse associated with Pharmacy may be "low," this case report highlights the dependence potential of this agent, as written in the approved product label: "[Pharmacy] has the potential to cause psychic and physical dependence of the morphine-type (�-opioid)."
